With check boxes clearly shown in each image, the user can simply mark the specific hazard, what P.P.E is worn at all times (CFR 1900.1200(g)), and which part of the body may be affected. Specifically, this label is designed for use with dangerous chemical or environmental hazards, and is noted in the top-right margin.
