This includes the backup for 30 years after staff is no longer employed with the office.  $50 each initial staff member. $35.00 each additional year (if Annual becomes required) Or $35.00 Additional if employee warrants a new fit test. 

Occupational Health Physician Clearance and Secure Backup of the Medical Questionnaire.  MSDS must Partner with Physicians in Occupational Health and Secure / Encrypt  the back up For 30 years past the Employees’ last day of work.  #M9922.  $50./ea staff first year.  Negotiating for additional years.  

For liability purposes as MSDS must partner with physicians in Occupational health and the secured back up that is necessary for such a long term.
